Description
This session will address how to help patients transition from oncology to primary care. The speaker will discuss the emotional and physical impact of this transition, follow up care of the patient and practical
methods to produce a positive transition.

Objectives

  • Identify appropriate populations for appropriate care management transitioning

  • Discuss tools available to help cancer survivors move through this process

  • Review opportunities within your organization to help survivors move from the oncologist to their primary care physicians appropriately
